Useful Forms

Integrated Working

A resource to support the delivery of multi-agency working.  It gives all practitioners working with children and young people access to resources and information about key aspects of multi-agency working including the Common Assessment Framework, Lead Practitioner and information sharing etc.

The Child In Need Procedures provide advice on agency responsibilities when making a referral

This booklet, Ten Pitfalls and how to avoid them,  aims to provide practitioners with a concise and accessible summary of research relevant to the initial stages of assessment in children's services. It has been written primarily for local authority practitioners undertaking statutory initial assessments and making Section 47 enquiries, but also has much broader relevance.

Stronger Families Teams

Stronger Families - Family Panels

The four multi-disciplinary Stronger Families teams include workers from health, social care, Troubled Families and the voluntary sector.  They engage with families at an early intervention level to offer support, which helps prevent the need to involve intensive, specialist services later on.  To further promote a multi-agency approach two of the four team managers are from a health background.

Each team operates a local Family Panel to consider families where a CAF has been undertaken, but the planned support is not achieving its objectives, and practitioners and families are unsure where to go next, or where families need a specific service which they're finding difficult to access.

For more information about the family panels, or if you have a family you'd like to bring to panel, contact the multi-agency co-ordinators:

  • Nigel Leeder for the west, central and north east  (07805914136)
  • Carol Ward for the south east (07805914142)
